That’s why we’re here.

For patients recovering from acute hospital care but not yet ready to return home, our Swing Bed program bridges the gap by providing short-term skilled nursing and rehabilitative therapy for conditions such as fractures, post-surgery recovery, or generalized weakness.

Our Swing Bed team consists of highly skilled healthcare professionals who work collaboratively to develop an individualized care plan that includes patient education. Patients participate in their care to help them achieve an optimum level of functioning.

Services Offered

The following services are offered through our program:

24-hour skilled nursing care
Care management
Dietician care
IV/Antibiotic transfusions
Occupational therapy

Physical therapy
Physician/Hospitalist care
Respiratory therapy
Speech therapy
Transitional/Discharge planning

Eligibility Requirements

To qualify, a patient must meet the following requirements:

Be insured by Medicare Part A or other insurance with hospital-based skilled care benefits.
A “three-consecutive night” hospital stay as an “Acute Inpatient” within the last 30 days.
An ongoing diagnosis and need requiring skilled care services related to the acute inpatient hospital stay that is typically provided in a skilled nursing facility.
